Who is eligible to enroll?
All enrollees must meet the following requirements to enroll:
  • All persons with a valid SSN or TIN
  • Primary & Spouse enrollees ages 18-64; unmarried child dependents under age 26
  • Primary residence in a US state where healthcare benefits are available for enrollment
  • Additional underwriting or provider requirements may exist based upon the plan selected.

When will I be billed?
Billing is a recurring monthly charge
For Individual/1099 enrollments - initial billing occurs at enrollment. Afterwards, you'll be billed on the 20th of each month for the next benefit coverage period.

For Group enrollments - invoicing is generated each month by the 15th, and payment is due on the 1st of the next benefit coverage period (month).

When do my benefits take effect?
Active Date for Benefit Plans
All benefit plans are 1st of the month effective. The cut-off date for enrollment is the 20th of the month prior to the desired effective date.

Example - enroll by January 20th for a February 1st effective date.

Is this major medical health insurance?
SmartHealth PrimeCare Plans
Are not major medical health insurance plans as defined by the Affordable Care Act (ACA).

By combining both insured benefits and non-insured benefits, SmartHealth plans help to reduce monthly rates and out of pocket costs to consumers by strategically allocating where costs and services are handled more effectively.

Are there network requirements?
Network Requirements
All non-comprehensive care outpatient services use the Multiplan PHCS network of doctors, urgent cares, and medical facities. https://providersearch.multiplan.com/sbmaspecificservices
Check PHCS Specific Services, then search.

Comprehensive care services, such as emergency room visits, hospitalization, and surgery do not have a network requirement.

What is a comprehensive care event?
Comprehensive Care Needs
A comprehensive care event is typically related to one of the following serious medical needs involving hospitalization; emergency room visit; surgery; catastrophic care or illness; or maternity.

In the event of comprehenssive care event or need, you can contact the comprehensive care advocacy team 24/7 by phone. The advocacy team will assist you with all aspects of comprehensive care event and can even assist with reducing or eliminating your out of pocket cost associated with your specific need.
